  • If you're unable to complete the paperwork in advance, please arrive 20 minutes early to fill it out in the office.

• We recommend:

  • minimizing jewelry, snaps, and zippers in your clothing

  • athletic wear

  • closed back shoes that are easily removable

  • tying up long hair
